This position is up to 3- 5 days a week with typical hours being 10 am to 4pm or 8am to 4pm, No weekends.

Applicants Must:

Forward Living will train any applicant that doesn’t have prior experience.

What a typical day for an SCC Mentor looks like:
You’ll meet people at our base site in the morning or you may provide transportation to some participants so they can meet others before heading out to enjoy activities in the community. Activities are determined by participants and facilitated by (SCC) Management. Activities could include volunteering at local non-profits, playing a game of pick-up basketball at a local park, touring a museum, going to the library, or enjoying a hike, to name a few. (SCC) Mentors are encouraged to develop and create new activities based on the interests of the people they serve as well as yourself, considering you will be in the community with them.

Sometimes you might spend the day at our office building with participants doing arts and crafts, playing board games or card games, facilitating a discussion group about current affairs, leading a cooking class, watching animal planet i.e. (Educational Content) or maybe even a dance party. Mostly these activities happen when the weather outside is very cold, usually under 35 degrees.

Variety is the spice of life, don’t be afraid to suggest a new activity that day program could try!

At the end of the day, you’ll ensure that everyone you were assigned to gets home safely.

Host Home Provider (HHP)

We support a diverse set of people with varying needs and are looking for homes and providers to match.

Forward Living is actively seeking energetic and professional Host Home Providers in the Colorado Springs areas to offer a home to adults with intellectual and developmental disabilities providing 24 hour care. Forward Living focuses on making lasting placements and will match an individual in services with you based on the compatibility of you, your family, and your home environment. Host Home Providers (HHP) offer an in-home placement to adults with disabilities of varying needs allowing each person to live in their own communities with the support they need to stay as independent as possible. Host Home Providers are contractors of Forward Living and receive competitive payments for the difficulty of care required for the person based on His/her Level scale. Forward Living provides comprehensive general training as well as specific training to the individual in care. Each (HHP) is specifically designed to meet the unique needs of an individual, respecting their personality, capabilities and hopes for the future. No two Host Homes look alike and we are flexible in developing a residential support plan that matches your needs with the needs of a person seeking placement. (HHP) are providing opportunities for individuals with intellectual disabilities to live in a home in their community.

Our residential team is on-call 24/7 and is there to support you if any needs come up during the placement. You become a part of our integrated care team and we aim to create placements that integrate fully into the community and your home.

Provider duties and responsibilities:

  • Provide personal assistance, social and emotional support, and other personal care to adults with intellectual and developmental disabilities in a home and community environment
  • Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed
  • Community activities based on the needs of the individual
  • Transportation services as needed – may include transportation to/from scheduled activities and appointments
  • Record Incident reports (IR)
  • Complete documentation for services provided
  • Medication administration and support as needed

Ultimately looking for people who are: Passionate about actively working to improve the lives of persons with disabilities – Desire to work closely within a community network of family members, individuals in services, and agencies – Willing to create a flexible, adaptive, and supportive environment to offer a home to an individual in need

Requirements for the contract:

  • Must be at least 21 years old
  • Must be able to lift at least 75 pounds
  • Must have a GED or diploma
  • Must have dependable and reliable transportation with the ability to safely transport
  • Must have a valid driver’s license
  • Must be able to pass background check
  • Must have appropriate living environment with at least one available bedroom
  • Preferred knowledge and understanding of developmental disabilities, mental health, Seizures, and trauma
  • Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained
  • Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at risk individual
  • Experience with Developmental Disabilities is preferred, but not required

Sample/Training Supports will provide:

  • General training on care giving and the needs of individuals with disabilities
  • Trauma-informed and social emotional care responses
  • First-aid, CPR, and general safety trainings
  • Person-centered training based on the person in care
  • Integrated and adaptive behavior support plans
  • Crisis intervention and emergency response planning
  • 24/7 on-call support
  • Support team of expert clinicians to provide in-home and community-based training as needed
  • Trained respite providers and proactive respite scheduling support
  • Protocols

Compensation:
Compensation varies depending on the level of support the individual needs. Offers a competitive rate ranging from $1200 – $6000 per month.
